La exóstosis del conducto auditivo externo (ECAE), también conocida como oído de surfista, es una alteración del tejido óseo del oído externo, principalmente causada debido a la sobrexposición al frío. Considerando que los practicantes de deportes acuáticos como el surf y bodyboard de las costas del pacífico sur se exponen a aguas con temperaturas entre 12-16 °C, se busca determinar, en este estudio, la prevalencia y grado de ECAE en practicantes de surf y bodyboard de las localidades de Reñaca y Concón durante el año 2018. Se realizó una evaluación del CAE mediante video-otoscopía a 67 personas (134 oídos) practicantes de surf y bodyboard de las playas de Reñaca y Concón, y se les aplicó un cuestionario respecto a sus hábitos de práctica. Como resultado, se observó una prevalencia de ECAE del 77,6%, siendo el 61,2% ECAE bilateral y el 16,4% ECAE unilateral. Se pudo determinar, además, que el 62,3%de los participantes no usa protecciones. A partir de estos hallazgos, es posible concluir que existe una alta prevalencia de la ECAE en practicantes de surf y bodyboard en las costas centrales chilenas, lo que debería alertar tanto a la población practicante como a profesionales de la salud auditiva con el fin de promover una vida saludable en esta población.


External auditory canal exostosis (ECAE), also known as surfer's ear, is an alteration of the bone tissue of the external ear, mainly caused due to overexposure to cold. Considering that those who practice water sports such as surfing and bodyboarding on the Pacific coast are exposed to waters with temperatures between 12-16 °C,. We seek to determine, in this study, the prevalence and degree of ECAE in surfers and bodyboarders from the coast of the south of Pacific Ocean, Reñaca and Concón, during the year 2018. An evaluation of the ECAE was carried out by means of video-otoscopy in 67 people (134 ears) who practiced surfing and bodyboarding from the beaches of Reñaca and Concón, and a questionnaire was used to collect information about their practice habits. As a result, a prevalence of ECAE of 77.6% was observed, with 61.2% bilateral ECAE and 16.4% unilateral ECAE. It was also possible to determine that 62.3% of the participants do not use protections. Based on these findings, it is possible to conclude that there is a high prevalence of ECAE in surfers and bodyboarders on the central Chilean coasts, which should alert both the practicing population and hearing health professionals to promote a healthy life in this population.

As exostoses maxilares são protuberâncias ósseas de caráter benigno, que se originam da cortical óssea e de etiologia controversa. Sua denominação vai depender da localização anatômica em que se encontra. Podem interferir na mastigação, fonação e adaptação de próteses removíveis parciais ou totais, devendo sua remoção ser considerada. O intuito deste trabalho é relatar o manejo de uma paciente com exostoses vestibulares em maxila e realizar uma breve revisão de literatura.


Maxillary exostoses are benign bony protuberances, which originate from the cortical bone and of controversial etiology. Its denomination will depend on its anatomical location. They can interfere with chewing, phonation and adaptation of partial or total removable prostheses, and their removal should be considered. The aim of this work is to report the management of a patient with vestibular exostosis in the maxilla and perform a brief literature review.

Background: Orthopedic diseases involving the forelimb fetlock joint of horses other than those used in professional racing have not been widely reported in the literature. One of these problems is the exostosis on the proximal forelimb phalanx (P1), which has not been reported previously in Western Libya. Aim: The study aimed to investigate the prevalence of P1 exostosis in traditional equestrian horses (known locally as Sarj horses) of different breeds in Libya that participated in a special social event and described its clinical findings radiographic appearance. Methods: The current study involved 60 horses admitted to private veterinary clinics with varied fetlock orthopedic problems. The studied horses were aged between 6 and 11 years old and galloped on hard grounds. Clinical and radiographic examinations were carried out on all horses. Lateromedial radiographs for one or both forelimb fetlock joints were acquired for each horse. Results: Clinical examination revealed that 21 horses (35%) had hard non-painful swelling on the dorsal aspect of fetlock with joint stiffness during flexion. In 19 of them, the swelling was bilateral. No forelimb lameness was noticed. Radiographically, hard swelling was diagnosed as bone exostosis, with various sizes, on the proximo-dorsal aspect of P1. There was also a new bone formation on the disto-dorsal part of third metacarpal bone in two cases. Conclusion: This study has documented the first report on bone exostosis on the proximo-dorsal aspect of the forelimb proximal phalanx as a common problem in Sarj horses that galloped on hard grounds in Western Libya. Although this bone lesion did not cause lameness in all reported cases, further investigations are warranted to identify its histopathological nature, potential etiology, and proper treatment.

La exostosis subungueal es un tipo de tumor óseo benigno, muy infrecuente en la infancia, que se localiza habitualmente en la falange distal del primer dedo del pie. Se presenta como una tumoración dolorosa que afecta a la uña y a los tejidos blandos periungueales. En su diagnóstico diferencial se incluyen lesiones dermatológicas como la verruga vulgar o el granuloma piogénico, y tumores óseos como el osteocondroma. El diagnóstico definitivo se basa en los hallazgos radiológicos e histopatológicos. Su tratamiento es la exéresis quirúrgica y suele tener buen pronóstico (AU)


Subungual exostosis is a benign bone tumor, very unusual in childhood, which is usually located in the distal phalanx of the first toe. It presents as a painful tumoration that affects the overlying nail and the periungual soft tissues. The differential diagnosis includes dermatological lesions such as common warts or pyogenic granuloma, and bone tumors such as osteochondroma. The final diagnosis is based on the radiological and histopathological findings. The treatment is surgical excision and the outcome is usually favourable. (AU)

RESUMO

Subungual exostosis is a benign, uncommon osteocartilaginous tumour that tends to recur. We here report the case of a 17-year boy with subungual exostosis, who reported a history of trauma. Treatment was based on direct surgery. The tumor was completely excised. The postoperative course was uneventful, with no recurrence identified.

CASE: Lesions, which commonly grow and protrude beneath the distal end of a toe nail and present to the podiatrist or foot and ankle surgeon, include subungual exostosis or enchondroma and a Nora lesion. Neurothekeoma is a benign dermal tumour of the peripheral nerve sheath that usually affects the skin of the head, neck, shoulders and arms. It occurs less commonly in the lower limbs and has only been reported twice in a subungual location. A case of subungual neurothekeoma that recurred twice due to inadequate margins of resection is presented. CONCLUSION: Although rare, neurothekeoma should be included in the differential diagnosis of a subungual lesion. Histopathological diagnosis is reached by differential immunostaining. Adequate clear margins of resection are recommended to prevent recurrence.

Haglund´s syndrome is characterized by osterosuperior calcaneal exostosis which irritates the Achilles tendon and manifests mainly as posterior talalgia. We here report the case of a 28-year-old woman on follow up for ankylosing spondylitis. Her mother was on follow up for the same systemic disease, and one of her maternal aunts had undergone surgery due to Haglund's syndrome. Patient's delay led to incorrect diagnosis as talagia was considered by physicians as a consequence of her system disease. The patient did not improve under medical treatment. Surgery outcome was satisfactory. This study raises a hypothesis about the genetic and hereditary character of Haglund's syndrome and its relationship with treatment response.

An eleven-year-old boy was seen by the general practioner due to a painfull hallux. The diagnosis unguis incarnatus was made. Therefore, a partial nail extraction was performed. Unfortunately, the patient still had complaints of a painful hallux and was forwarded to the surgical outpatient clinic. We saw a round, hard and benign tumor on the lateral side of the nail (picture 1). We performed a partial nail extraction as well. Due to persistence complaints and the abnormal course and X-ray was performed. On the X-ray an exostosis was seen (picture 2). This confirmed the diagnosis: subungual exostosis. It is a rare clinical manifestation and has a long delay up until diagnosis. It is a benign bone tumor and often seen on the hallux. A radical excision must be performed.

RESUMO

Exostosis is a type of benign bone tumor in which trabecular (spongy) bone overgrows its normal border in a nodular pattern. When the growth occurs under the nail bed, it is termed subungual exostosis or Dupuytren exostosis. This condition may mimic other bony abnormalities such as an osteochondroma and may present with nail deformities with or without pain. For this reason, a biopsy of the lesion is necessary to rule out a precancerous growth. In rare cases, pediatric patients may have subungual exostosis, as demonstrated in our case.

Nail pathologies have a broad range of origin and may sometimes be complicated in presentation or clinical course, specifically when the pathology remains recalcitrant after treatment. In this case report we discuss a pathologic disorder that was initially misdiagnosed as a pyogenic granuloma surrounding an ingrown nail but was later found to be a benign neoplastic bone growth, Dupuytren exostosis, also known as a subungual exostosis. Operative treatment was deemed appropriate for the patient, and the exostosis was resected, leaving a soft-tissue void at the distal toe. The remaining void was filled with a perinatal graft, the use of which has been deemed effective anecdotally in both chronic and acute lower-extremity wounds but has not been widely discussed in the lower-extremity literature. This graft was placed to aid in wound healing over a potentially difficult wound bed. As amniotic, chorionic, and umbilical grafts become more prevalent in lower-extremity surgery, its antitumor effects should be further explored and published. This is the first case report, to our knowledge, of the successful use of a perinatal graft in the setting of a bone tumor, and it demonstrates that certain benign neoplasms can be treated with resection and placement of a perinatal graft while helping to prevent chronic wounds at surgical sites.

RESUMO Introdução: Exostose é um crescimento ósseo benigno que comumente acomete a mandíbula e a maxila, porém raramente apresenta grandes dimensões. Objetivo: Descrever um caso de exostose palatina de apresentação rara e discutir os principais aspectos clínicos envolvidos. Relato de caso: Paciente, gênero masculino, 66 anos, melanoderma, com história de aumento de volume no palato duro com tempo de evolução indeterminado, sem queixas álgicas, porém relatava transtornos de mastigação e fonação. O conjunto da lesão obliterava toda a profundidade do palato e ocupava grande volume na cavidade oral. A Tomografia Computadorizada de Feixe Cônico demonstrava áreas bem corticalizadas na superfície da lesão e áreas centrais menos hiperdensas, compatíveis com osso medular maduro. Após exames e pareceres pós-operatórios o paciente foi submetido a tratamento cirúrgico sob anestesia geral para remoção da massa óssea e osteoplastia subjacente, a fim de dar contornos normais ao palato, e o material colhido foi enviado para a biópsia que estabeleceu o diagnóstico de exostose. O paciente evoluiu sem complicações. Conclusão: Embora a exostose seja um crescimento ósseo benigno, ela causou problemas funcionais ao paciente, necessitando assim de um tratamento cirúrgico. Após três meses da remoção, os distúrbios foram revertidos à normalidade, e a sua reabilitação protética está em andamento(AU)


RESUMEN Introducción: La exostosis es un crecimiento óseo benigno que generalmente afecta la mandíbula y el maxilar, pero rara vez es grande. Objetivo: Describir un caso de exostosis palatina rara y discutir los principales aspectos clínicos involucrados. Presentación del caso: Paciente masculino de 66 años, melanoderma, con antecedentes de aumento de volumen del paladar duro con una evolución de tiempo indefinido, trastornos de masticación y fonación El conjunto de lesiones borró toda la profundidad del paladar y ocupó un gran volumen en la cavidad bucal. La tomografía computarizada de haz cónico mostró áreas corticalizadas en la superficie de la lesión y áreas centrales menos hiperdensas, compatibles con la médula ósea madura. Después de los exámenes y consejos preoperatorios, el paciente se sometió a un tratamiento quirúrgico con anestesia general para eliminar la masa ósea con osteoplastia subyacente a fin de obtener contornos del paladar normales, y el material recolectado fue enviado para realizar biopsia que estableció el diagnóstico de exostosis. El paciente progresó sin complicaciones. Conclusiones: Aunque la exostosis es un crecimiento óseo benigno, causó problemas funcionales al paciente, por lo que requirió tratamiento quirúrgico. Tres meses después de la extracción, los trastornos han vuelto a la normalidad y su rehabilitación protésica está en proceso(AU)


ABSTRACT Introduction: Exostosis is a benign bone growth that commonly affects the jaw and maxilla, but is rarely large. Objective: Describe a case of rare palatal exostosis and discuss the main clinical aspects involved. Case presentation: A case is presented of a male 66-year-old dark-skin patient with a history of hard palate enlargement of an indefinite time of evolution, and chewing and phonation disorders. The lesions had obliterated the entire palatal depth and occupied a large section of the oral cavity. Cone beam computed tomography revealed corticalized areas on the surface of the lesion and less hyperdense central areas compatible with mature bone marrow. After preoperative examination and advice, the patient underwent surgical treatment under general anesthesia to remove the bone mass and underlying osteoplasty to obtain normal palatal contours. The material collected was sent for biopsy testing, which established the diagnosis of exostosis. The patient recovered without any complications. Conclusions: Although exostosis is a benign bone growth, in this case it caused functional problems to the patient, thus requiring surgical treatment. Three months after removal, the disorders have been reverted to normal and prosthetic rehabilitation is underway(AU)

Subungual exostosis is a rare benign osteocartilaginous tumor, of unknown etiology, that affects the subungual part of the distal phalanx and predominantly affects adolescent females. The objective of this study is to evaluate the results of surgical treatment of subungual exostosis in pediatric patients by means of surgical excision and nail preservation. We present a retrospective study, including pediatric patients with subungual exostosis who underwent surgical treatment in a single institution, over a 6-year period, We describe the surgical technique used, the characteristics of the patients and lesions, and evaluate the results obtained in terms of function, satisfaction and complications. Thirteen patients were included in this study, seven female (54%) and six male (46%), median age 11 years old. The most common location was the hallux, in eight patients (73%). Patients had good functional results with a median American Orthopedic Foot & Ankle Society score of 100 [80; 100] and a median personal satisfaction score of 9. The median follow-up was 30 [3;68] months. In our patients, subungual excision with nail bed preservation, allows a good balance between the radical excision of the exostosis and the preservation of the natural protection granted by the native nail, yielding good functional results and patient satisfaction.

Posterior ankle impingement is a syndrome characterized by discomfort or pain at the hind foot during plantarflexion. The etiology can be divided into three main categories: overuse, trauma and anatomic abnormalities. Regarding overuse, usually patients that complain of posterior ankle pain are ballet dancers, downhill runners, field athletes and soccer players secondary to flexor hallucis tendinitis.

OBJECTIVE: Various anatomical structures of upper airway and physical differences are known to be risk factors for obstructive sleep apnoea (OSA). Torus mandibularis is a structure that can appear on the inside of the mandible. Therefore, it is possible for tori to influence airway volume by occupying the space for tongue and cause sleep apnoea. The purpose of this study is to investigate the effect of torus mandibularis on the severity of OSA as one of the craniofacial risk factors. DESIGN: Retrospective case-control study. SETTING: University-based tertiary medical centre. PARTICIPANTS: Adult patients over 19-years-old who visited outpatient clinics with complaints of sleep-disordered breathing symptoms between January 2010 and December 2017 were investigated. MAIN OUTCOME MEASURES: The presence of torus mandibularis in oral cavity was confirmed by physical examination or CT image. We analysed demographic findings including age, sex, medical history, previous operation history, physical findings of upper airway and result of polysomnography. To evaluate the effect of torus mandibularis on OSA, polysomnography data of the two groups according to presence or absence of torus mandibularis were compared and analysed. RESULTS: Two-hundred and thirty-two OSA patients with BMI <25 were divided into two groups, according to either the presence or absence of torus mandibularis. We analysed 138 patients of control group and 94 of torus mandibularis group. Apnoea-hypopnoea index (AHI) was 18.8 ± 14.9 in control group and 25.1 ± 18.4 in torus mandibularis group (P = 0.006). Respiratory disturbance index (RDI) was 23.1 ± 14.7 in control group and 27.9 ± 18.4 in torus mandibularis group (P = 0.035). Supine AHI showed 26.6 ± 20.3 in control group and 32.5 ± 22.6 in torus mandibularis group (P = 0.039). Patients with torus mandibularis had a trend of increase in proportion according to the severity of sleep apnoea, such as AHI (P = 0.007) or RDI (P = 0.034). CONCLUSIONS: We newly found that the presence of torus mandibularis affects not only severity of OSA and also position-dependent OSA. These results support the necessity of torus mandibularis evaluation in OSA patients, and further study is also required to investigate its consequence in the surgical outcome.
